Diabetes Care Program
The Diabetes Care Program consists of registered nurses, dietitians and nurse practitioners who work in consultation with other health professionals in a multi-disciplinary approach to the management of the person with diabetes. Services provided through Diabetes Care support adult and pediatric populations.
We facilitate the patient/family education program by conducting education sessions and providing ongoing counseling and support. This promotes a better understanding of diabetes management for the person with diabetes and their families, in both inpatient and outpatient settings.
We serve as a resource to health care workers and the public. We aim to decrease the incidence of Type 2 diabetes and complications of diabetes in the central region through health promotion and effective management.
What should I bring to my appointments?
Please bring all your medications, your glucometer and a record of your blood sugar readings.
